7th - 12th grade students planning to participate in a sport during the 2025-2026 school year, please use the attached physical form.  There are also forms available in the high school main office.  We will NO LONGER be using Planet HS.  Please complete pages 1 through 5, and have your physician complete page 6.  

All completed physicals can be dropped off to the high school main office Monday through Thursday, from 6:00 am to 4:00 pm.  Thank you!
